Parents of children who will be five years old by August 1, 2009, must register for kindergarten screening by bringing the child’s birth certificate, social security card, immunization record and custody papers, if applicable, to the Westview Kindergarten Center located at 401 Scott Avenue. Children will not need to come during the registration process. Please call 419-673-4294 to schedule an appointment.

Immunization records must be presented at the time of registration.  The State of Ohio requires that all students entering kindergarten have 5 DTaP, 4 Polio, 3 Hepatitis B, 2 MMR and Varicella.  If your child has had natural chickenpox, and presents a signed statement from the parent, guardian, or physician to that effect, they are not required to be immunized against chickenpox.  Any student without these immunizations will be directed to the school nurse or the Kenton-Hardin Health Department.

During registration, copies of the required paperwork will be made, parents will fill out school information forms, and a date and time for the actual screening of the child will be given.
