The Kenton Hardin Health Department will be performing a vaccine clinic for the Kenton Middle School on Friday – May 14th, 2010 from 9:00 A.M. until 10:45 A.M. at Kenton Middle School. With the recent Ohio Department of Health’s requirement for entry into 7th grade for the fall of 2010 that all 7th graders must have a tetanus booster to enter school this fall, this will assist the parents in getting this accomplished before school starts.
There will be registration forms sent home with all 6th graders this week for the parent/guardian to complete, sign and return to the Kenton Middle School NO LATER THAN MONDAY-MAY 10, 2010.
The parent does not need to be present for the vaccine to be administered on 5-14-2010 but must have completed and signed the consent for this.
The cost of the vaccine is $10.00 payable that day. If you are sending a check please make it payable to the Kenton Hardin Health Department. If your child is covered under Medicaid, the registration form that you are to complete will require this information.
